Pre-Consultation Questionnaire

The pre-consultation questionnaire is a crucial step in our healthcare process, allowing our professionals to evaluate your specific travel requirements, address concerns, and provide important information about potential health risks at your destination. To get started, follow these simple steps:

REGISTER

ONLINE

Click on the link below to access our secure portal.

 

If this is your first time using our services, you will need to register.

Please note that each patient should complete their registration individually.

STEP 3

COMPLETE QUESTIONNAIRE

Once you have logged in, select the service you desire and then “Start” the consultation. For travel vaccinations, select “Travel Health.” Once you have completed the pre-consultation uestionnaire, make a note of your patient ID.

STEP 4

ATTEND YOUR APPOINTMENT

Simply attend your scheduled appointment at the designated date and time.
